 HIPAA Authorization Release Form

In order for Desoto Imaging Specialists to disclose protected health information (PHI) to any individual and/or entity outside of normal treatment, payment or health care operations as defined by Federal guidelines, a signed HIPAA Authorization Release form must be received from the patient or legal guardian. How We Bill

Memphis Physicians Radiological Group, PC (MPRG) is the physician group that will bill your insurance company or you for the services you recieve. MPRG utilizes a third party billing company called Affiliated Professional Services, Inc. (APS) who will send statement to you and/or your insurance carrier. Should you have billing questions you may contact APS at 800-299-9770.
If you should have a biopsy you will receive a separate bill from the pathologist.
